Consent and Cosmetic Declaration - Client Information

I confirm that I have disclosed all relevant medical conditions and, to the best of my knowledge, remain fit to receive acupuncture and massage treatments. I agree to inform my practitioner of any changes to my health or concerns regarding my suitability for treatment.

I understand the nature of physical contact during treatment and acknowledge my right to withdraw from any session at any time should I feel uncomfortable.

In the event of a medical emergency, I authorise the practitioner or responsible persons to seek appropriate medical assistance on my behalf. I accept responsibility for any reasonable decisions made in this context and trust their judgement under such circumstances. I also authorise the practitioner to use their first aid or CPR training if necessary.

I acknowledge that while complementary therapies may offer physical or emotional benefits, no guarantees or claims are made regarding treatment outcomes. I accept full responsibility for the results of my treatment and understand that Kallos Acupuncture and Mariah Sayer are not liable for any adverse effects or reactions.

I understand that these therapies are not a substitute for medical care, and I am advised to consult with a GP or healthcare professional regarding any medical concerns.

I understand that acupuncture and related therapies (including acupressure, cupping, microneedling, massage, aromatherapy, and electrostimulation) may cause temporary discomfort, marks, or bruising due to their nature.

Cosmetic Declaration

I understand that while objective improvement may be achieved through treatment, results can be subjective and may vary between practitioner and patient. It is my responsibility to discuss my expectations, recognising that lifestyle factors can influence outcomes.

I acknowledge that facial cosmetic treatments may cause temporary localised bruising, particularly around the eye area due to the delicate vasculature. Minor capillary bleeding and visible bruising may occur and are not harmful to the treatment outcome. In some cases, bruising is an unavoidable part of achieving optimal results.

By booking and attending a treatment with Kallos Acupuncture, I confirm that I have read, understood, and agreed to the terms of this Consent & Cosmetic Declaration. I acknowledge my responsibility to inform my practitioner of any changes to my health or circumstances that may affect my suitability for treatment.
